By signing the registration form you are acknowledging and are agreeing to all the terms below.(Please initial by each that you have read them)

The registration fee of $25.00 and first month tuition is due at the time of registration.

You are committed to a $ monthly tuition fee (August09 -May 2010)

Tuition is due by the 5th of every month regardless of school closings or illness. A $10 late fee will be added to any account that is not paid by the 5th.

$25.00 check fee on all returned checks.

$55.00-$65.00 costume fee is due by November 1, 2009, info to come.

All fees are nonrefundable under any circumstances.

The dance season’s tuition is divided into 10 equal installments. The amount of any single installment is not based on the number of classes for that month, it merely represents one tenth of the season’s tuition.

Acknowledgement of Risk and Waiver of Liability

I understand that there may be some risk of injury associated with participation in dance and/or gymnastics classes and I waive any and all claims of liability, release and hold harmless Kidz in Step in the event that such an injury may occur to my child. In the event of an accident when parent, Legal guardian, or emergency contact are not available, I give permission to Kidz In Step to procure medical attention.
